Following your Initial Consultation with Dr. Schoonover and discussing your individualized treatment plan, Natalie (our Practice Administrator) will be checking on your insurance policy deductible and whether treatments are covered. Typically, most insurers cover the larger, varicose vein treatments for symptomatic, chronic venous insufficiency. Many insurers do not cover surface vein treatments. After verifying coverage/benefits with your insurance company, you will then receive a cost-estimate via phone/email.
According to the contract Indiana Vein Specialists® has with your insurance company, network discounts MAY NOT apply to non-covered services. This includes calendar year benefits that may be exhausted.
After your initial statement, our policy is for payments to be made within 30 days for the balance due. This amount will be reflected on your statement. Please make payments upon receipt of your initial statement to avoid additional charges. Payments can be made by sending in the payment stub or by phone at 317.348.3023.
Please be prepared to provide your insurance card(s) prior to each appointment. Please inform us at check-in of any insurance changes, such as a new group or policy number, co-payment, or new employer. All insurance policies must be presented prior to being seen, including any secondary coverage.
