To assist with preparing for your first visit, we greatly appreciate you completing the Indiana Vein & Lymphatic New Patient Registration forms prior to coming in for your appointment. You will receive a welcome text or email from us with a link to complete your New Patient Paperwork.

Key Information for New Patient Visits

Before Your Visit: You will receive a reminder call from our office. You will also receive a welcome text or email with links to your new patient registration paperwork.

Paperwork: Complete your forms online ahead of time (New Patient Information). If you can’t finish them online, please arrive 1 hour early. That gives our team time to enter your information and prepare it for your provider’s review.

What to Bring: A copy of your insurance card, your driver’s license or state ID, and your compression garments (if you wear them).

Insurance: We will verify your insurance benefits at your first consult visit.

Pre-Visit Prep: Please drink plenty of water and eat a good meal before your appointment.

Childcare Policy: For patient safety, please leave little ones at home. Our treatment rooms are small, and our team needs to focus entirely on your visit. We may need to reschedule your visit if kids tag along!

Payments: Copays and any amounts due are collected when you check in.

Rescheduling: If you need to change your appointment, please give us at least 48 hours’ notice so we can offer the time to another patient waiting to be seen.

Feeling Sick? If you have a fever, cough, or flu symptoms, call us ahead of time so we can reschedule you.

After your first visit, if you would like access to the Patient Portal, please request the office to send you an email with a username and temporary password to access the Patient Portal.
